Guaranteed Hours: 36 hours/week
Pay Package: $2,830 Weekly
• $44/hr W2 Pay
• $1,246 Weekly Stipend

Requirements

Mandatory Requirements to Apply

• Active Vermont RN License (Required at submission)
• Minimum 1 year of clinical nursing experience
• Home Health experience preferred
• Strong patient assessment and care planning skills
• Experience with EMR systems (Homecare Homebase/HCHB preferred)
• Ability to work independently and manage multiple patient cases

Roles & Responsibilities

• Provide professional nursing care to patients of all ages in a home health setting
• Complete admissions, evaluations, and develop individualized care plans
• Monitor patient progress and adjust care plans as needed
• Collaborate with homecare teams and paraprofessionals for coordinated patient care
• Maintain accurate documentation and ensure compliance with healthcare standards
