Long Term Care - LPN

As an LPN in long-term care, you provide ongoing chronic disease management, daily nursing care, and routine assessments for residents in assisted living or nursing facilities.

The role emphasizes person-centered care, safety, and coordination with a multidisciplinary team to maintain residents' quality of life.

Responsibilities
  • Monitor vital signs, administer medications, and document responses over time.
  • Perform wound care, dressing changes, and routine procedures per care plans.
  • Assess changes in condition and communicate with RNs, physicians, and families.
  • Assist with activities of daily living and rehabilitation activities as directed.
  • Maintain accurate records in the electronic health record and ensure safety protocols.
Qualifications

Graduate of an approved practical nursing program.

Current LPN license in the state of practice.

  • CPR/BLS

1+ year of long-term care or skilled nursing experience; strong communication with families and team members.

Experience
  • Experience delivering ongoing care in long-term care settings
  • Experience with care planning and documentation in an EHR
Preferred
  • Experience with wound care or palliative care principles
Work Environment

Facility-based long-term care setting with rotating shifts; collaborative team of nurses, CNAs, and therapists.

Documentation in the EHR; regular coordination with physicians and family members.
