Occupational Therapist

Provide personalized, one-on-one occupational therapy to older adults in private homes and senior living communities, enhancing mobility, independence, and quality of life through evidence-based treatments. Responsibilities include conducting evaluations, developing care plans, delivering therapy sessions, documenting in electronic medical records, and collaborating with interdisciplinary teams of PTs, SLPs, and fitness specialists. Educate patients, caregivers, and staff to promote safety and aging in place, integrating telehealth and virtual care as appropriate for increased flexibility and ongoing support. Qualifications Valid OT license or eligibility in the practice state Degree from an accredited OT program Proficiency with computers and EMR systems Current CPR certification Physical Requirements Ability to exert medium effort, assist with patient transfers, reposition patients, stand and walk for extended periods, climb stairs, drive to patient locations, and wear PPE when needed.
