Job Description:

Identify the patient's and family's physical, psychosocial, environmental, safety, and developmental needs and individualize the patient's plan of care based on the needs identified. Document problems, nursing assessments, appropriate goals, care provided, care interventions, and patient and family outcomes achieved from intervention and care provided.

Responsibilities:

• Assess all aspects of the patient's pain and develop an individualized pain management plan.
• Anticipate, prevent, and treat undesirable symptoms or secondary symptoms.
• Participate in the interdisciplinary team (IDT) meeting to establish, review, and revise the plan of care for each patient.
• Maintain up-to-date patient records to ensure that problems, plans, interventions, and goals are accurately and clearly stated.
• Teach appropriate interventions, medications, effects, and side effects to the patient, caregiver, volunteers, and others as appropriate.
• Implement care to achieve outcomes of comfortable and safe dying, self-determined life closure, and effective grieving for patient and family.
• Responsible for maintenance of electronic medical records, assuring accuracy, completeness, and compliance with regulations, certification standards, legal and ethical standards.
• Support, instruct, and educate the patient, family, and caregiver.
• Coordinate all patient and family services.
• Maintain the dignity of the dying patient and support the patients' and family's unique spiritual and cultural beliefs.
• Provide holistic, family-centered care across treatment settings to improve the quality of life.
• Consult and collaborate with the interdisciplinary team and others involved in the patient's care.
• Provide care consistent with National Hospice and Palliative Care Organization standards of practice for hospice programs.

Requirements:

• RN Hospice Case manager needed for Fort Worth office.
• Monday-Friday 8A-5P, hospice experience is required.
• Experience with HCHB is required.
