Job Title: Primary Care Physician
Location: North Hollywood, CA
Job Type: Full-time
Work Setting: Senior primary care and community-based care
Schedule: Monday through Friday, generally 8:00 AM to 5:00 PM, with limited rotating phone-only call
Pay: $275,246 to $330,595 base salary, plus quarterly bonus potential up to 10%
Overview
A mission-driven senior care organization is hiring a Primary Care Physician in North Hollywood, CA. This role is ideal for a physician who wants to provide thoughtful, relationship-centered care for medically and socially complex older adults in a collaborative, value-based care model.
The position offers regular weekday hours, low daily patient volume, no hospital rounds, and the opportunity to work closely with an interdisciplinary care team focused on whole-person support.
Responsibilities
- Provide comprehensive primary care for older adult patients in a center-based and community-based setting.
- Complete in-person assessments and develop individualized care plans.
- Collaborate with an interdisciplinary team to support medical, behavioral, functional, and social needs.
- Partner with advanced practice providers and other clinical team members on complex cases.
- Support care transitions across acute, short-term, long-term, and home-based settings.
- Complete home visits and skilled nursing facility visits as needed.
- Participate in care planning discussions and make clinical recommendations.
- Participate in limited rotating phone-only call coverage.
Required Qualifications
- MD or DO from an accredited program.
- Successful completion of an ACGME-accredited residency program.
- Active, unrestricted physician license in California preferred.
- State CDS certification and DEA registration.
- Eligible to provide services to Medicare and Medi-Cal participants.
- Ability to obtain and maintain CPR and First Aid certification within 180 days of hire.
- Strong clinical judgment and comfort caring for complex older adults.
- Collaborative, humble, team-oriented approach to care.
Preferred Qualifications
- Board certification preferred.
- 2 or more years of clinical experience preferred.
- Background in Family Medicine, Internal Medicine, Emergency Medicine, or Geriatrics.
- Geriatrics fellowship is a plus.
- Experience in value-based care, senior care, community-based care, or interdisciplinary care models.
- Experience collaborating with nurse practitioners, physician assistants, and broader care teams.
