Residential Services Coordinator/ICMS Case Manager

Job Type Full-time Description Provides support to staff and coordinates program collaboration among partner agencies, public agencies, property management, and other key partners. Organizes and manages internal program operations, including monitoring daily functioning of Life skills, Educational and Employment Services for residents enrolled at project-based sites. Delivers services on-site and off-site, referring residents to intensive case management, life skills workshops, money management, educational attainment, health services, counseling, job training and placement, and resident activity coordination. Essential Duties and Responsibilities Ensure housing performance standards of partners and funders are achieved including a minimum of 90% housing retention rate. Provide individual and team support to engage residents to workgroup being offered onsite or offsite. Ensure adherence to funding source contracts and performance standards; prepare written reports on program activities as assigned and as required by funding sources. Participate in meetings related to Homeless TAY, Families and Single Adult Services in LA County. Develop, enhance, and expand relationships with other agencies and public entities to support TAY, Families and Single Adults. Ensure that services to residents are reflective of CRCD model, which emphasizes housing first model, harm reduction, and client’s centered approach. Provide support to staff as needed, including in emergencies and crisis situations to cover gap between staff transition. Be flexible to work evening and weekend as needed to participate in Resident Community Meetings and occasionally events scheduled during the weekends. Conduct a minimum of one face-to-face contact with each resident on a bi-monthly basis. Conduct quarterly meetings with a designated representative of the primary service provider for each resident to discuss tenant’s progress. Act as a liaison among the Program Manager, the property management company, resident, and the primary mental health provider to facilitate the communication of concerns that could jeopardize the resident’s residency. Provide information and instructions to residents regarding Fair Housing, tenant rights and responsibilities, including observation of rental agreement and property rules. Act as a liaison and coordinate linkage to services with numerous public social service agencies and community providers, three categories will assist them in achieving and sustaining independence: employment and education/ training activities, money management counseling and legal and benefits counseling. Ensure residents are obtaining rental assistance: move-in and eviction prevention services. Address tenant grievances, mediate resident disputes and resolve any issues with property management that might affect the client’s tenancy. Enforce housing rules in collaboration with property management. Development of workshop opportunities on relevant topics including housekeeping, money management, parenting, daily living skills (including transportation, training in self-advocacy/community accessibility) needed to maintain housing and achieve independence. Monitor status of tenants who have moved into housing and prepare tenants who are deemed ready to transition to mainstream non-supportive housing. Uphold CRCD’s Mission Statement and 5 Year Strategic Plan. Qualifications A bachelor’s degree in related field, or an equivalent combination of education and experience. Minimum of four years’ full-time experience working with the homeless, preferably those living with mental illness and/or substance addictions. Detail oriented with excellent time management, organizational, written, verbal, interpersonal, and computer skills. Able to work in a high tolerance Harm Reduction model with clients who have multiple barriers. Self-Starter with ability to stay ahead of the curve and thrive in a fast-paced environment. Strong Interpersonal and team building skills. Experience working with homeless youth, single adults, and families. Reliable, flexible collaborator who works well with minimal supervision, who has a decent work ethic, and an ability to set and maintain personal boundaries. Demonstrated ability to work on multiple projects simultaneously, and to meet deadlines. Reliable transportation: valid driver’s license, and car insurance as required by law, as appropriate.
