Position Summary
The Community Manager is responsible for driving census growth, developing referral relationships, generating new client opportunities, and serving as the market ambassador for the organization. This role focuses on building professional partnerships, increasing community visibility, and achieving revenue growth goals through new client acquisition.
Key ResponsibilitiesBusiness Development & Referral Growth- Build and maintain relationships with hospitals, rehabilitation facilities, senior living communities, physicians, social workers, discharge planners, hospices, and other referral sources.
- Conduct regular visits, presentations, and networking activities to generate referrals.
- Develop and execute territory growth plans.
- Track referral activity and conversion rates.
- Identify opportunities to expand services within current referral accounts.
- Represent the organization at networking events, community events, and professional associations.
- Coordinate educational presentations and in-service programs.
- Promote the company's services and value proposition.
- Maintain visibility within assigned territories and communities.
- Meet monthly targets for:
- New client starts
- Revenue growth
- Referral source development
- Qualified prospect generation
- Collaborate with operations to ensure smooth client onboarding.
- Follow up with prospective clients and families.
- Maintain ongoing communication with referral partners.
- Address concerns and ensure referral partner satisfaction.
- Provide regular updates to leadership regarding market trends, opportunities, and threats.

- Bachelor's degree preferred.
- 3+ years healthcare marketing, business development, home care, hospice, senior living, or related experience.
- Strong networking and relationship-building skills.
- Excellent communication and presentation abilities.
- CRM proficiency preferred.
- Reliable transportation for community visits.
