Rehabilitation & Long-Term Care Pharmacist

Support patient recovery and independence through personalized medication plans.

Key Responsibilities:

Review and monitor complex regimens in skilled nursing or rehab facilities.
Coordinate care with therapists, physicians, and caregivers.
Educate patients and families on medication goals and outcomes.

Qualifications:

PharmD with LTC, SNF, or rehab pharmacy experience preferred.
Compassionate communicator with strong organizational skills.
Familiarity with Medicare Part D regulations.
