Responsibilities:

• Provide comprehensive physical therapy assessments and interventions in a home setting.
• Develop and implement individualized treatment plans, including home exercise programs.
• Educate patients and families on medication, safety, injury prevention, and management of medical conditions.
• Document patient progress and maintain accurate records using BayCare Forms and ACE Timesheet system.
• Collaborate with healthcare team members to optimize patient outcomes and ensure quality care.
• Adhere to OSHA, HIPAA, and safety protocols, including COVID-19 and flu vaccination requirements.
