Job Summary

We are seeking an experienced Hospice RN Case Manager to provide compassionate, patient-centered care for individuals receiving hospice services. The RN Case Manager will manage an assigned caseload, coordinate interdisciplinary care, and support patients and families through end-of-life care planning and symptom management.

Minimum Requirements

✅ Active RN License
✅ Minimum 4 years of recent Hospice RN or Case Management experience
✅ Strong assessment, care coordination, and patient education skills
✅ Experience managing hospice patient caseloads preferred
✅ Ability to work independently and collaboratively with interdisciplinary teams
