Hospice Social Worker — End-of-Life & Family Support

Aurora Health Care

Aurora Health Care is seeking a Social Worker in Burlington, Wisconsin, to provide psychosocial evaluations, ongoing counseling, and community referrals to meet the needs of terminally ill patients and their families. The ideal candidate will have a Master's Degree in Social Work, a relevant state license, and experience in medical or hospice settings. This role requires critical intervention skills and effective collaboration within interdisciplinary teams.
