Licensed Master Social Worker (LMSW) Part-Time / PRN Home Health & Hospice Agency
- Conduct comprehensive psychosocial assessments of patients and families.
- Evaluate social, emotional, financial, environmental, and community support needs.
- Develop and implement individualized care plans in collaboration with the interdisciplinary team.
- Provide counseling, crisis intervention, and emotional support to patients and caregivers.
- Assist patients and families in accessing community resources, financial assistance programs, and support services.
- Educate patients and families regarding available healthcare, hospice, and community services.
- Facilitate advance care planning and discussions regarding patient goals and preferences.
- Participate in interdisciplinary team meetings and contribute to patient care planning.
- Maintain accurate, timely, and compliant clinical documentation.
- Coordinate discharge planning and transitions of care when appropriate.
- Provide bereavement support and grief counseling to families and caregivers.
- Ensure compliance with federal, state, agency, Medicare, and hospice regulations.
- Maintain patient confidentiality in accordance with HIPAA requirements.
- Perform other duties as assigned by the Clinical Director or Administrator.
- Master's Degree in Social Work (MSW) from a program accredited by the Council on Social Work Education (CSWE).
- Current Licensed Master Social Worker (LMSW) license in the State of Texas.
- Current driver's license and reliable transportation.
- Current automobile liability insurance.
- Minimum one (1) year of social work experience preferred.
- Experience in Home Health, Hospice, Healthcare, Case Management, Geriatrics, Palliative Care, or Medical Social Work preferred.
- Knowledge of Medicare, Medicaid, and community resources preferred.
- Strong assessment, counseling, and crisis intervention skills.
- Excellent verbal and written communication abilities.
- Ability to work independently and as part of an interdisciplinary team.
- Strong organizational and time-management skills.
- Compassionate and empathetic approach to patient care.
- Proficiency with electronic medical records (EMR) systems.
- Knowledge of hospice philosophy and end-of-life care principles preferred.
- Ability to travel to patient homes, facilities, and community locations.
- Ability to sit, stand, walk, bend, and lift up to 25 pounds occasionally.
- Ability to utilize a computer, tablet, and other office equipment.
- Part-Time and PRN positions available.

Join our Home Health and Hospice team as a Licensed Master Social Worker (LMSW). The LMSW provides psychosocial assessments, counseling, resource coordination, grief support, and advocacy services for patients and families. Candidates must possess a Master's Degree in Social Work (MSW), an active Texas LMSW license, reliable transportation, and excellent communication skills. Home Health or Hospice experience is preferred but not required. Flexible Part-Time and PRN schedules available. License/Certification: Texas LMSW License (Required) Education: Master's Degree in Social Work (MSW) (Required) Work Location: Field-Based/Home Visits Job Types: Part-Time, PRN Experience: Home Health, Hospice, Medical Social Work, or Case Management preferred.
