Nurse Practitioner or Physician Assistant Primary Care - Skilled Nursing Facilities

Description Community Health Centers of America (CHCA) is seeking an experienced, dedicated and compassionate Physician Assistant (PA) or Family Nurse Practitioner (FNP) to join our growing team in a full‑time (5 days/week) or part‑time (3 days/week) position.
