Territory San AntonioNew Braunfels Come join our team at Three Oaks Hospice and our sister companiesAgape Hospice Care Sage Hospice Primary and Palliative Care Elevation Hospice of Colorado Elevation Hospice and Primary and Palliative Care of Utah We are growing and looking for compassionate professionals who want to make a meaningful impact while building a rewarding career in hospice and palliative care Together we share a unified mission to deliver best in class care to patients and families While each organization maintains its own identity and local culture we operate as one connected networkusing shared systems and support to create a smooth consistent and candidate friendly hiring experience Why Work for Us We are committed to being an employer of choice offering a supportive culture centered on patient care clinical excellence and employee success If youre looking for purpose stability and growththis is the place to be Join our team Position Summary Primary function is to provide coordinated care in the homes to patients of all age groups and perform psychosocial assessments analysis counseling and referrals to meet the needs of the patientfamily Essential Duties Assesses and completes the Comprehensive Assessment within five days of the patients election of hospice care to identify patient and familys psychosocial financial environmental and community needs as evidence by POC documentation clinical records IDGIDT meetings and community resource referrals Assists with advance directives funeral planning discharge planning family counseling and may assist in changes in the level of care as indicated by the patient or familys needs Uses effective interpersonal communication skills both verbal and written Meets mandatory continuing education requirements of the Hospice and licensing board Demonstrates commitment professional growth and competency by maintaining a working knowledge of public and private eligibility standards and requirements Provides patient and familycare giver counseling around issues of end of life death and grief Promote hospice philosophy and administrative policies to ensure quality of care Attends IDGIDT meetings participate in the patient care planning process and coordinating and collaborating with theIDGIDT to promote coordination of patient care Accepts clinical assignments that are consistent with education and competence to meet the needs of the patients Provides an environment which promotes respect for patients and their privacy and property Participates in the Hospices QAPI program Demonstrates commitment professional growth and competency Follows the Hospice infection control policy Promotes the Hospices philosophy and administrative policies to ensure quality of care Participates in the Hospices educational in service programs as require QUALIFICATIONS Bachelors or masters degree in social work from a State accredited program If has a bachelors degree and is hired after 12022008 must be supervised by a Master of Social Work Current and valid license as a LMSW Working knowledge of community resources Good interpersonal skills Hepatitis profile Current State Certification Reliable transportation Valid Drivers License and current auto liability insurance #J-18808-Ljbffr
