Care Plus Hospice Care LLC in the Phoenix AZ valley seeks an After Hours Hospice Nurse (On-Call) to provide expert clinical care and compassionate support to patients and families during their most critical moments after hours. You will respond to after-hours calls, perform skilled nursing assessments, manage pain and symptoms, document in EMR, and coordinate with the on-call physician and interdisciplinary team.
