Optum Home & Community Care, a part of the Optum family, is seeking a Nurse Practitioner to join our HouseCalls team serving Holmes, Jackson, and Washington counties in Florida. This field-based role focuses on annual in-home health assessments for Medicare Advantage and other plans, in collaboration with primary care providers.
