to join our Homecare team. This role offers the flexibility to work around your summer schedule while providing one-on-one patient-centered care within the community. As an RN Home Health Clinician, you'll manage a caseload of patients from admission through discharge while collaborating with a dedicated interdisciplinary team to support positive patient outcomes.

What You'll Do

• Perform comprehensive patient assessments and develop individualized care plans
• Coordinate care and referrals across disciplines and specialties
• Monitor patient progress and adjust care plans as needed
• Educate patients and caregivers on managing care at home
• Communicate effectively with patients, families, and healthcare providers
• Maintain accurate clinical documentation
• Support continuity of care and quality patient outcomes

Qualifications

• Current Rhode Island Registered Nurse (RN) license required
• Bachelor's Degree in Nursing or related field strongly preferred
• Minimum of 1 year clinical nursing experience required
• Home health or acute care experience preferred
• Current CPR certification
• Valid driver's license and automobile liability insurance
• Strong communication, organization, and clinical assessment skills
