We are seeking a Family Medicine Physician / Primary Care Physician to join an established, patient-centered medical practice in Crestview, Florida . This is an excellent opportunity for a physician seeking long-term stability, strong clinical support, and the ability to practice evidence-based medicine in a collaborative care environment.

You will work alongside experienced providers, nursing staff, and leadership in a team-based care model focused on delivering high-quality primary care services to the community.

Medical Insurance (Capital Health Plan or Florida Blue)
Employer pays 80% of employee-only coverage
$25,000 life insurance (100% employer paid)
Long-term disability (60% of salary up to $7,500/month employer paid)

One state license renewal paid
DEA renewal paid
One professional society membership paid

Retirement & Time Off:

403(b) retirement plan with employer contributions up to 5%
100% vested after 3 years
days PTO based on tenure
8 paid holidays + 2 floating holidays

Continuing Education:

Up to 5 CE days annually
$2,500 annual CE allowance

Physician Responsibilities:

Provide comprehensive primary care services for patients of all ages
Diagnose and treat acute and chronic medical conditions
Manage preventive care, screenings, and immunizations
Develop and implement patient-centered treatment plans
Order and interpret laboratory and diagnostic tests
Prescribe medications and treatment therapies
Provide patient education and follow-up care
Refer patients to specialists and community resources when appropriate
Participate in care team meetings and quality improvement initiatives
Maintain timely and compliant electronic medical record documentation

Qualifications:

M.D. or D.O. from an accredited medical school
Board Certified or Board Eligible in Family Medicine
Board certification required within 3 years if board eligible
Active Florida Medical License or ability to obtain
Current DEA registration
Current CPR certification
Eligible for malpractice coverage
