Nurse Navigator-Cardiology/Heart Failure Job Category: Clinical Requisition Number: NURSE013018 Apply now Posted: April 29, 2026 Type: Full-Time Location: Fishersville, VA 22939, USA 78 Medical Center Drive, Fishersville, VA The Cardiology/Heart Failure RN Navigator assists patients in navigating care from first contact with the AMG Cardiology department through ongoing care with an AMG cardiologist. Responsibilities include coordination of post‑hospital care, scheduling appointments and procedures to prevent readmission, collecting required documentation, and reporting. The RN Navigator collaborates with the inpatient team, cath lab, and clinic to coordinate care planning and problem‑solve issues related to system, financial, or social barriers. They actively participate in the discharge process, providing teaching, medication assistance, and scheduling post‑procedure clinic visits. The RN Navigator also participates in internal and external quality assurance programs related to cardiology and heart failure, including hospital accreditation and certification expectations. Education, Licensure/Certification, and Experience Requirements Nursing Diploma or Associate Degree in Nursing required BSN preferred Current and valid Registered Nurse license in Virginia or a Compact state required BLS required ACLS within six months of employment required Certified Heart Failure Nurse (CHFN) or Cardiac Vascular Nursing Certification (RN‑BC) recommended within two years of hire Minimum of two years critical care, cardiology, cardiac cath lab, or heart failure clinic experience required Experience with cardiology and/or heart failure patients preferred Job Duties Ensure hospital follow‑up appointments are scheduled for patients discharged from the hospital Act as liaison between patients, medical staff, and services Plan and implement collaborative programs with referral services for seamless delivery of care Identify and assist patients who may qualify for financial assistance or prescription drug programs Monitor inpatient consults to provide patient education on new diagnoses, medications, and chronic disease management, such as heart failure Assist with post‑hospital discharge medication coordination Consult community resources to optimize treatment and support Respond to patient calls or urgent referral cases Assist with appointment and test scheduling for pre‑transplant patients Contact patients discharged from Cardiology service within 72 hours and document calls in the clinic EHR system Collaborate with the ACO case management team regularly Perform other duties as assigned by Management Report and Record Keeping Assist in tracking patient interventions and outcomes Participate in research as appropriate Track patients as they move through the healthcare system Provide timely and complete documentation for accurate patient care Collect and report heart failure patient data related to accreditation and certification Coordinate and assist with Augusta Health Foundation grants related to cardiology or heart failure Create quarterly and annual reports as needed Participate in developing, implementing, and evaluating policies, procedures, and quality assurance programs Customer Service/Marketing Proactively resolve issues to improve quality and efficiency of the department(s) Consistently display professionalism with patients and peers Embrace new ideas and procedures that improve departmental performance Initiate opportunities or collaborate with other departments Provide assistance in a clear, courteous, professional, and flexible manner, both orally and in writing Work cooperatively with staff in a team environment Assist in marketing plan collaboration with the Marketing Department Keep supervisors and team members informed of work progress Show competence and leadership skills meeting standards Benefits Generous paid time off to promote work life balance Free onsite parking Shift and weekend differentials Onsite child care Company Information Augusta Health is a mission‑driven, independent, nonprofit, community health system located in Fishersville, VA, in Virginia’s scenic Shenandoah Valley.
