Physical Therapist (PT) - Outpatient Therapy at the Riverfront

Overview Evaluates and treats patients of all ages, modifying plan of care as appropriate. Serves as a role model in delivery of professional services and as a clinical resource for staff and students. This role qualifies for a sign-on bonus and relocation if moving 50+ miles! Qualifications Bachelor's Degree in Physical Therapy Licensed Physical Therapist (PT) – Illinois Department of Financial and Professional Regulation (IDFPR) Basic Life Support (BLS) certification within 30 days – American Heart Association (AHA) Complete all required continuing education regarding the care of acute stroke patients Responsibilities Perform patient transfers with assistance as needed; maintain safe body mechanics. Show respect and empathy toward patients, colleagues, and staff. Design and implement a plan of care that integrates goals, treatments, outcomes, and discharge plans; adjust care based on patient response and evidence. Educate patients, family, caregivers, students, and other health care providers using effective teaching methods. Evaluate clinical findings to determine physical therapy diagnoses and outcomes of care. Conduct examinations using reliable and valid methods relevant to the chief complaint, screening results, and history. Produce documentation per the Clinical Process Policy (CTSCP67). Screen patients to determine the effectiveness and need for therapy services when appropriate. Utilize support personnel within legal and ethical standards; monitor their care and provide feedback. Complete annual education as required for DNV Hip and Knee Replacement Program compliance, when applicable. Evaluate impairments related to orthopedic and neurologic conditions. Establish a plan of care based on functional deficits and patient goals. Complete objective testing for range of motion, strength, and performance of daily functional tasks. Document and charge daily in the therapy EMR. Apply taping techniques for stabilization, facilitation, or inhibition of movement. Use therapeutic modalities such as ultrasound, interferential, or iontophoresis as appropriate. Communicate with referring providers regarding patient progress and treatment recommendations.
