Pilgrim Tower East

158-unit senior residential community in Pasadena CA

. In this role you will help identify resident needs connect residents with community resources coordinate supportive services and develop programs that promote health wellness independence and quality of life. The ideal candidate is compassionate organized resourceful and comfortable building strong relationships with residents service providers and community partners. What Youll Do

Resident Services & Advocacy Assess resident needs and connect residents with appropriate social health wellness and community resources. Provide referrals support and advocacy while promoting resident independence and self-advocacy. Provide information and assistance during crisis situations and conflict resolution. Service Coordination & Programs Build relationships with community agencies and service providers to expand resources available to residents. Coordinate and monitor resident referrals follow up on services and maintain a current directory of community resources. Develop and coordinate educational health and wellness programs based on resident needs. Serve as a liaison between residents management staff and community service providers. Documentation & Reporting Maintain accurate and confidential records of resident interactions referrals and services. Prepare daily and monthly reports related to resident services and activities. Additional Duties Perform other duties as assigned in support of RHFs mission and the needs of the community. Qualifications

Bachelors degree in Gerontology Human Services Social Work Psychology Sociology or a related field is preferred. Ability to speak read and write Chinese is preferred. Relevant work experience supporting older adults or individuals with disabilities will be considered. Knowledge of supportive services and community resources for older adults. Strong advocacy problem-solving communication and interpersonal skills. Ability to maintain confidentiality and demonstrate sound ethical judgment. Ability to work effectively with residents staff service providers and community partners. Strong organizational and time-management skills with the ability to work independently. Proficiency with Microsoft Office including Word Excel PowerPoint and Outlook. Physical Requirements

Ability to move throughout the community and interact with residents and service providers. Ability to communicate effectively in person and by phone.
