Position Purpose:

Under the direction of the Chief Executive Officer (CEO), the Chief Operating Officer (COO) is the senior operating executive of Aviva Health, accountable for the day-to-day performance of the organization’s clinical service lines and the operational infrastructure that supports them, including but not limited to Family Medicine, Specialty Care, Dental, Behavioral Health, Clinical Support Services, and Pharmacy. The COO translates the strategic direction set by the CEO and Board of Directors into executable operating plans, budgets, staffing models, and performance targets, and holds operational leaders accountable for results. Working in close partnership with the Chief Medical Officer (CMO), the Chief Financial Officer (CFO), and other members of the executive team, the COO drives access, productivity, quality, patient experience, and fiscal sustainability across all Aviva Health sites.

The COO ensures continuous operational compliance with HRSA Health Center Program requirements, licensure, and applicable accreditation standards. The COO represents Aviva Health in the community and with external stakeholders including coordinated care organizations, hospital and specialty partners, and State, Regional, and Federal agencies, and serves as a principal advisor to the CEO and Board of Directors on operational strategy and acts on behalf of the CEO as designated. The COO develops and guides organizational leaders throughout Aviva Health and is a key architect of the organization’s leadership bench.

Essential Functions:

• Directs the day-to-day operations of all Aviva Health clinical and operational departments, including Family Medicine, Specialty Care, Dental, Behavioral Health, Clinical Support Services, Child Advocacy and Pharmacy.
• Directly supervises the Directors of Clinical Operations for Medical and Dental, the Vice President of Behavioral Health (operational line), and other functional leaders as assigned; establishes accountability structures, performance expectations, and development plans for the operational leadership team.
• Serves as the operational half of the executive dyad with the CMO; is jointly accountable with the CMO for clinical department performance, provider experience, and the effectiveness of the department-level dyad partnerships. Supports and develops dyad pairs so that operational and clinical leaders work from shared goals and metrics and resolves escalated dyad conflicts jointly with the CMO.
• Oversees, monitors, trains, evaluates, and corrects personnel assigned to Operations, assuring personnel adhere to the policies, standards, norms, and procedures of Aviva Health.
• Translates the Board-approved strategic plan into annual operating plans with measurable objectives, timelines, assigned owners, and resource requirements; reports progress to the CEO and Board of Directors.
• Standardizes workflows, staffing models, and operating policies across sites and service lines while preserving the flexibility required by each department.
• Accounts for the development, management, and execution of operating and capital budgets across multiple functional departments; partners with the CFO on forecasting, variance analysis, and corrective action.
• Manages provider and staff productivity, panel size, schedule template utilization, visit volume, and access metrics to sustain both mission delivery and fiscal margin.
• Evaluates operational contracts, vendor relationships, capital requests, and service agreements within delegated authority, and develops business cases for new investment.
• Leads operational planning for facilities, equipment, space utilization, and site development in support of organizational growth.
• Ensures compliance with the HRSA Health Center Program Compliance Manual within areas of the COO’s responsibility, hours of operation, after-hours coverage, and continuity of care requirements.
• Serves as a standing member of the Quality Improvement (QI) Committee, representing the operational and clinical workflow perspective in QI planning and prioritization.
• Partners with the CMO and QI leadership to translate approved QI initiatives into clinical operations — the workflow, staffing, and scheduling changes needed to execute them. The COO works collaboratively with the QI team to implement the operational changes needed to maintain compliance and performance on both.
• Oversees the operational components of risk management, patient safety, infection prevention, emergency preparedness, continuity of operations, and patient grievance resolution.
• Acts on behalf of the CEO in their absence, as designated.

Qualifications:

• Bachelor’s degree required.
• Master’s degree in healthcare administration, business administration, public health, nursing, or a related field strongly preferred.
• Minimum of seven (7) years of progressively responsible experience in healthcare operations, clinic management, or healthcare-related leadership, including at least three (3) years at the director level or above with responsibility for multiple departments. FQHC or community health center experience strongly preferred.
• Demonstrated experience supervising senior leaders in a complex, multi-site, multi-disciplinary setting.
• Experience operating within a dyad leadership model, or demonstrated ability to share accountability with clinical leadership and influence outcomes without direct authority over clinical staff.
• Working knowledge of revenue cycle operations, including patient access and front desk workflow, call center performance management, eligibility and enrollment, charge capture, and denial management.
• Working knowledge of the HRSA Health Center Program — including the Health Center Program Compliance Manual, FTCA deeming, UDS reporting, sliding fee discount programs, and 340B requirements — or the demonstrated capacity to attain proficiency within the first year in role.
• Strong financial analysis, budget planning, and management experience and skills, including operating and capital budget development, cost-per-visit and productivity analysis, payer mix and revenue cycle fundamentals, and business case development.
