Join our team as a Weekend Warrior RN, supporting patient care from Friday through Monday. This full-time position offers flexibility in scheduling. For example, a typical schedule may start around 2:30 PM on Friday and continue through Monday morning (around 10:00 AM), but we are open to adjusting hours to fit the right candidate while maintaining weekend coverage. Schedule, Hours & Territory Weekend coverage from Friday through Monday with flexible start and end times to fit your schedule. This role includes on-call responsibilities, along with completing scheduled visits as needed. Territory can span from West Branch to Gaylord, MI. Key Responsibilities Conduct comprehensive patient assessments and develop individualized plans of care. Provide skilled nursing care in accordance with hospice standards and physician orders. Coordinate care with physicians, caregivers, and interdisciplinary team members. Educate patients and families on symptom management, disease progression, and end-of-life care. Document assessments, interventions, and care plans accurately and in a timely manner in the EMR HCHB (Homecare Homebase). Participate in interdisciplinary team meetings and care planning. Support after-hours patient needs as part of an on-call rotation when applicable. Requirements Active Registered Nurse (RN) license. Hospice or home health experience preferred. Strong clinical assessment and care coordination skills. Ability to work independently in the field. Strong communication and documentation skills. Experience using HCHB (Homecare Homebase) preferred. Field Role Requirements Active driver's license. Ability to pass a criminal background check. Ability to pass a Motor Vehicle Record (MVR) check.
