Minimum Requirements

Bachelor’s, Master’s, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA or equivalent accrediting body. Current, unencumbered South Carolina physical therapy license required to start the assignment. Current CPR (BLS) certification required. Experience in home health or community-based care preferred. 2-3 years of physical therapy experience preferred. Familiarity with electronic documentation and basic computer navigation required. Must have reliable transportation and a valid driver’s license for home visits.

Assignment Details

Facility type: Home Health. Shift name: Days. Schedule: 5x8. Role expectations: Perform patient evaluations, provide therapeutic services per physician orders, supervise and evaluate PTAs and home health aides as assigned. Must be able to perform home visits and evaluate the home environment as part of the plan of care.
