Job Title Travel Physical Therapist – Grand Rapids, MN Overview Travel assignment of 13 weeks beginning May 25, 2026. 8‑hour day shifts, 40 hours per week in a skilled nursing facility. Responsibilities Evaluate residents and develop individualized therapy plans. Provide hands‑on treatment to improve mobility and function. Collaborate with the care team to coordinate patient care. Document patient progress and update care plans. Qualifications Graduation from an accredited physical therapy program. Active Minnesota Physical Therapist license. Minimum of one reference in full work history. Experience working in a skilled nursing facility (preferred). Availability to begin on the specified start date.
