The Clinical Pharmacist reviews and interprets medication orders, monitors drug therapies, identifies opportunities to optimize treatment, and provides evidence-based recommendations to support the best possible patient outcomes. You'll also serve as a trusted pharmacy resource, providing education and clinical guidance to pharmacy team members and healthcare professionals.

Responsibilities:

• Accurately interprets, profiles and files prescribers' orders. Ensures all medications filled, checked and delivered in a timely manner.
• Monitors drug therapy for duplicates, contraindications, interactions, incompatibilities, allergies, and dosing errors. Documents interventions in the electronic health record (EHR) professionally and accurately.
• Reviews and monitors drug therapy that requires dosage adjustment per prescriber order. Intervenes with prescriber to clarify medication orders, dosage, frequency, route as needed. Documents changes in EHR as per verbal or telephone order.
• Aseptically prepares intravenous fluids, irrigation solutions, additives, chemotherapy and other sterile products. Maintains appropriate documentation. Dispenses control drugs and maintains accurate records and inventory as required by law and pharmacy policies.
• Maintains inventory of pharmaceuticals and supplies, and ensures the proper storage condition. Participates in stock control programs (ordering, inventory and monitoring usage) as required.
• Obtains emergency drug items or out of stock items as needed per pharmacy policy and reviews all non-formulary requests for formulary alternatives prior to dispensing.
• Ensures correct medications are filled, dispensed and delivered appropriately.
• Oversees pharmacy technicians in accomplishing daily tasks and requirements set forth per pharmacy policy and procedures.
• Defines problems, collect data, establish facts and draw valid conclusions, interpret technical instructions and deal with abstract variables.
• Collaborates with nursing to ensure safe and effective medication use is being performed, available for questions, clarifications, safeguarding/troubleshooting automated dispensing cabinets and medication pumps in a professional manner.
• Provides medication and disease state education to patients, both face-to-face during the patient's hospitalization and/or via telephone communication (when necessary).
• Performs all other duties as assigned by management within job scope.

Qualifications:

Education / Training:

• Graduate Pharmacy

Preferences: Doctor of Pharmacy Degree with a completed PGY residency in pharmacy practice.

Experience Requirements:

• 1-year Pharmacist experience

Preferences: 2 years of hospital clinical pharmacist experience

Certificates/Licenses/Registration:

• Licensed Pharmacist in Florida
• Basic Life Support (BLS)
• Advanced Cardiovascular Life Support (ACLS)

Additional Information: BLS, provided by the American Heart Association (AHA), required within 30 days of hire. ACLS, provided by the American Heart Association (AHA), required within 90 days of hire not required for PRN employees.
