- Additional Info: We are a rapidly growing, entrepreneurial not-for-profit Federally Qualified Health Center and social service agency that serves the homeless and unstably housed people in New York City.DETAILS:- Performs history and physical assessments as indicated by his/her specialty area- Develops a plan of treatment/care for the patient that also includes education and follow-up visits- Must have a commitment to serving the underprivileged, as well as be flexible and have the ability to function in a nontraditional setting- EMR = eClinicalworks- Role will require some supervision of mid-level providers
