We are excited to announce a sign-on bonus of $5,000! The Sub-Acute and Long-Term Care sector is rapidly growing, offering excellent potential and a flexible work-life balance (no on-call duties or weekend shifts)! We are looking for a Full-Time provider (32-40 hours per week) for our Lebanon Center situated in Lebanon, NH. This role is open to either a nurse practitioner or a physician assistant, and we encourage new graduates to apply. The annual salary for this position ranges from $112,320 to $124,800, along with an uncapped incentive compensation bonus program.

Key Responsibilities

• The Advanced Practice Provider will report directly to the Director of Clinical Practice and/or the Area Clinical Practice Manager.
• This role involves collaborating on-site with the Center Leadership Team to meet center-specific goals and priorities.
• Collaboration with the Medical Director and/or Attending Physician is essential for delivering patient-specific medical care.
• The Advanced Practice Provider will offer primary and urgent care services to residents of the nursing center, working alongside other medical providers.
• Collaboration with Center Leadership is crucial to fulfilling our mission of enhancing lives through high-quality healthcare and compassionate service.

Qualifications

• The Advanced Practice Provider must have graduated from an accredited School of Nursing, hold a registered nurse license, and be certified as a Nurse Practitioner with a Master’s Degree in Nursing, focusing on Older Adult and/or Family Nurse Practitioner (FNP) specialties. Other concentrations may be considered based on experience and organizational needs.
• The Physician Assistant must have completed a program accredited by the ARC-PA and passed the PA National Certifying Exam (PANCE) to obtain the PAC designation. State licensure and NCCPA certification are required.
• A minimum of two years of experience working with adult or geriatric populations is preferred.
• In states where practice regulations permit, APRNs should hold an unrestricted professional license and DEA for each state of practice. PAs must also have an unrestricted professional license and DEA for their respective states.

  • Average annual bonuses of approximately $13,000 through our uncapped Incentive Compensation Program (for providers working 40 hours per week).

Note: Restrictions may apply based on collective bargaining agreements, state laws, and factors such as pay classification, job grade, location, and length of service. To qualify for full-time benefits, a minimum of 30 hours per week is required. Salary Range: $112,320.00 - $124,800.00 per year. Bonus: $5,000.00
