Behavioral Health Care Manager (MSW/LCSW)
Family Care Health Centers
Family Care Health Centers seeks a Behavioral Health Care Manager (MSW with LMSW/LCSW preferred) to lead care management within an integrated primary care and behavioral health team in Missouri. The role includes patient enrollment, case management, and inter-provider coordination. Three years of social work or behavioral health experience is required; urban health setting experience is a plus.
