Position: Home Care Coordinator Discharge Planner – specializes in coordinating home care services and resources for patients transitioning from hospital to home, acting as a discharge planning resource without providing hands‑on clinical care. Job Requirements Graduate of an accredited nursing or allied health care professional program. BS preferred. Licensure in clinical discipline (RN) in the State of Maryland & Washington DC. Active CPR certification. Work Experience Two (2) years of home care or home care discharge planning experience preferred. Knowledge of Medicare Conditions of Participation. Thorough knowledge of home care community resources, agency policy and procedures, federal and state regulations and third‑party regulations for reimbursement of home care services.
