Licensed Physical Therapist Assistant (PTA) - Full-Time | Milwaukee, WI Location: Milwaukee, WI Facility: Sunrise Health Services Setting: Skilled Nursing Facility (SNF) Full-Time Physical Therapist Assistant Opportunity Continuum Therapy Partners is hiring a Licensed Physical Therapist Assistant (PTA) for a full-time position at Sunrise Health Services in Milwaukee, Wisconsin. This opportunity allows you to work with adult and geriatric patients in a skilled nursing and rehabilitation setting while partnering with an experienced interdisciplinary therapy team dedicated to improving patient outcomes. Whether you’re an experienced Physical Therapist Assistant or a recent graduate, you’ll have the opportunity to grow your career while helping patients improve mobility, strength, balance, and functional independence. Physical Therapist Assistant (PTA) Responsibilities Provide skilled physical therapy treatments under the supervision of a licensed Physical Therapist. Assist patients with gait training, therapeutic exercise, balance activities, strengthening, transfers, and functional mobility. Monitor patient progress and communicate changes to the supervising Physical Therapist. Complete accurate and timely electronic documentation. Collaborate with physicians, nurses, Occupational Therapists, Speech-Language Pathologists, and the interdisciplinary rehabilitation team. Educate patients and caregivers on mobility techniques, safety, and home exercise programs. Maintain compliance with Medicare, state, federal, and company guidelines. Qualifications Graduate of an accredited Physical Therapist Assistant program. Current Wisconsin Physical Therapist Assistant (PTA) license required. Skilled nursing, long-term care, rehabilitation, post-acute care, or outpatient experience is preferred but not required. New graduate PTAs are encouraged to apply. Strong communication, teamwork, and organizational skills.
