We are seeking an autonomous clinician with a background in geriatric, adult, or family care, who is comfortable managing complex chronic conditions. The ideal candidate will be a collaborative team player, eager to contribute to an interdisciplinary approach to patient care, and possess a strong commitment to improving the quality of life for long-term care residents. ## Key Responsibilities - Provide wound care to long-term care residents in skilled nursing facilities - Assess and manage acute changes in condition; initiate appropriate diagnostic workup and treatment - Conduct advanced care planning and manage chronic disease conditions - Prescribe medications including controlled substances in accordance with Kentucky law - Communicate with nursing home staff and collaborate with attending physicians on care plans - Perform comprehensive wound assessment, including NPIAP staging - Utilize advanced wound products and manage wound vacs ## Qualifications - Current, unrestricted Nurse Practitioner license in Kentucky (required) - National board certification (AANP or ANCC) - Active DEA licensure - Clinical background in adult, family, or geriatric specialties - Demonstrated experience in wound assessment, NPIAP staging, and advanced wound products - Experience with wound vac management - Wound certification preferred - Experience in long-term care or skilled nursing facility setting preferred - Proficiency with electronic medical records (EMR) ## Benefits - Continuing education credits - Recertification reimbursement - Malpractice insurance coverage - Bonus pay - Mileage reimbursement - Disability insurance Schedule - Part Time-based schedule serving skilled nursing facilities in Flatwoods, KY Location Flatwoods, KY 41139 Flatwoods, Kentucky, a charming city in Greenup County, offers a sparse suburban feel with a population of over 7,000.
