Under the direction of a licensed Physical Therapist, the PTA will provide skilled therapy services designed to help residents improve mobility, strength, function, and independence. Key Responsibilities Provide physical therapy treatments as directed by the supervising Physical Therapist Implement individualized treatment plans based on physician orders and therapy goals Monitor resident progress and communicate changes to the Physical Therapist Document treatment sessions, resident responses, and progress notes accurately and timely Assist with discharge planning and caregiver education Collaborate with nursing staff and interdisciplinary team members to achieve positive resident outcomes Participate in care conferences and team meetings as needed Maintain a clean, safe, and organized therapy environment Ensure compliance with all state, federal, and facility regulations Support residents in achieving their highest level of function and independence Qualifications Education & Experience Graduate of an accredited Physical Therapist Assistant program Experience in skilled nursing, rehabilitation, or long‑term care preferred Experience with PointClickCare (PCC) and Casamba preferred Strong documentation, communication, and organizational skills Licenses & Certifications Current South Carolina Physical Therapist Assistant (PTA) license in good standing Must maintain all required continuing education requirements Must remain in good standing with the State Board What Makes You a Great Fit Compassion for an underserved population Strong clinical and interpersonal skills Positive attitude and team‑first mindset Dependable, flexible, and self‑motivated Passion for helping residents achieve their highest level of independence We are an equal opportunity employer.
