Care Manager Assistant (Per Diem)

$25 per hour

Job Summary Reporting to the leadership of Care Coordination, in collaboration with the VP of Population Health, this position serves as a liaison to connect patients and families to the correct level of service in the healthcare system to ensure optimal health outcomes. Collaborates closely with members of the care team to identify and reduce barriers to care; assist families in locating and accessing resources; and educate families on the importance of accessing care promptly. Pay Range Minimum $25.00 | Midpoint $26.00 | Maximum $26.00. Rate of pay is determined by various factors, including but not limited to knowledge, skills, competencies, experience, education, as well as position requirements. Experience Minimum Required: 1 year administrative or clerical experience. Preferred: 2‑3 years administrative or clerical experience in a medical office setting or hospital‑based environment with patient and family interaction. Knowledge of medical terminology, insurance verification, and authorization processes. Education Minimum Required: High School diploma or equivalent. Preferred: Bachelor’s degree in a related field. Training Minimum Required: Training. Preferred: Training. Special Skills Ability to work effectively under pressure due to changing priorities, interruptions, high/low census, payer demands, and discharge planning demands. Excellent interpersonal communication, including conflict resolution, problem solving, and negotiation skills. Clinical knowledge of multiple age groups, medical illness and treatment plans according to disease process, diagnosis and anticipated LOS. Strong organizational and time management skills, as evidenced by capacity to multi‑task and prioritize. Ability to work independently and exercise sound judgment in interactions with physicians, payers, patients and their families. Demonstrates the knowledge and skills necessary to communicate with third party payers to obtain authorization for the appropriate treatment setting for patients requiring medical, psych, and chemical dependency treatment. Must be proficient in required competencies upon completion of orientation and maintain annual departmental competency requirements. Ability to work collaboratively regardless of social, economic and cultural backgrounds. Computer skills in using Microsoft Office (Excel, Word, Power Point, Access). Preferred: Knowledge of CCS/Medi‑Cal regulations and requirements. Knowledge of third‑party payor requirements to optimize reimbursement. Preferred: Bilingual. Licensure Minimum: Licensure. Preferred: Licensure. Work Environment - Functional Demand Medium to high energy level. Lift and carry 40‑50 lbs. Push/pull 150‑200 lbs. (Patients on bed, stretcher). Lateral transfer 150‑200 lbs. (Patient). Physical Activity Requirements Lifting (Floor to waist level) – Constant 67% or more. Lifting (Floor to waist level) – Frequent 36–66%. Lifting (Floor to waist level) – Occasional 0–35%. Lifting (Waist level and above) – Constant 67% or more. Lifting (Waist level and above) – Frequent 36–66%. Lifting (Waist level and above) – Occasional 0–35%. Bending, carrying objects, climbing, keyboard use/repetitive motion, pinching/fine motor activities, push/pull, reaching forward, reaching overhead, sitting, squat/kneel/crawl (Squad & Knee), standing, talking or hearing, tasting or smelling, twisting, walking, wrist position deviation. Sensory Requirements Color discrimination. Depth perception. Far vision. Hearing. Near vision. Environment Requirements Airborne communicable disease. Bloodborne pathogens. Chemical. Dust particulate matter. Extreme noise levels. Extreme temperatures. Radiation. Uneven surfaces or elevations.
