Position: Nursing (TheraCare Home Health – Houston – Stafford, TX 77477) – PRN. Role involves administering skilled nursing care under RN supervision at patients' homes, coordinating interdisciplinary care, and preparing for discharge.

Qualifications

• Licensed Vocational Nurse, per state requirements.
• Minimum 1 year of Home Health experience.
• Good communication, assessment, and clinical documentation skills.
• Ability to read, write, and speak English.
• Reliable transportation and current auto liability insurance.

Essential Functions

• Under RN direction, assist in identifying the patient’s physical, psychosocial, and environmental needs.
• Participate in planning and implementing care with RN per plan of care.
• Provide care using infection control measures that protect staff and patients.
• Record treatments administered in medical record.
• Record clinical documentation per accepted regulatory, corporate, and professional guidelines.
• Participate in weekly case conferences.
• Participate in interdisciplinary communication to ensure patient needs and expectations are understood and optimized.
• Provide verbal or written patient status updates to supervising RN before reassessment, supervisory and/or discharge visits, and as indicated by patient response to treatment.
• Participate in in-service training programs.
• Responsible for assuring patient safety.
• Comply with state regulations and national standards.
• All documentation completed and synced by 9:00AM the following day, 7 days per week.
