RN - Case Manager

Job Opportunity

This role involves the coordination of patient care management, ensuring optimum utilization of resources, service delivery, and compliance with external review agencies. The successful candidate will provide ongoing support and expertise through comprehensive assessments, care planning, plan implementation, and overall evaluation of individual patient needs.

Assist in enhancing the quality of patient management and satisfaction.

Promote continuity of care and cost-effectiveness through the integration of case management, utilization review, and discharge planning functions.

Serve as a patient advocate by effectively utilizing resources, planning discharges, and addressing the holistic needs of patients.

Collaborate with interdisciplinary teams to ensure effective patient care strategies.

Must possess BLS certification from the American Heart Association.

Minimum of 2 years of recent RN Case Management experience in LTAC or STACH settings.

Experience with MCG, discharge planning across acute care settings, and strong interdisciplinary team collaboration.

Active RN license in California is required.

This position is scheduled for Monday to Friday from 8:30 AM to 5:00 PM.
