Individual Support
- Assist individuals with activities of daily living (ADLs), including personal hygiene, bathing, dressing, grooming, toileting, and meal preparation.
- Support individuals in developing and maintaining independent living skills.
- Promote individual choice, self-determination, and person-centered outcomes.
- Provide companionship and emotional support.
- Assist individuals with participating in community activities, social events, recreational opportunities, and employment-related activities.
- Provide transportation to appointments, work, community outings, and other scheduled activities as needed.
- Encourage the development of natural supports and community relationships.
- Administer medications and complete medication documentation as trained and authorized.
- Monitor and report changes in health, behavior, or overall well-being.
- Implement Individual Service Plans (ISPs), Behavior Support Plans, and other support strategies.
- Ensure a safe and clean environment while adhering to agency policies and DODD regulations.
- Respond appropriately to emergencies and critical incidents.
- Complete accurate and timely documentation, including service notes, incident reports, medication records, and daily logs.
- Maintain confidentiality and protect individual rights in accordance with HIPAA and agency policies.
- Participate in required trainings and maintain all certifications.
- Attend team meetings and communicate effectively with supervisors, families, guardians, and other service providers.
Education
- High School Diploma or GED required.
- Previous experience working with individuals with developmental disabilities preferred but not required.
- Strong communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Compassion, patience, and commitment to supporting individuals with disabilities.
- Ability to follow written and verbal instructions.
- Basic computer and documentation skills.
- Must be at least 18 years of age.
- Valid driver's license and acceptable driving record.
- Ability to pass background checks required by DODD and agency policies.
- Must maintain or willing to obtain required certifications and training, including CPR/First Aid and Ohio-specific training requirements.
- Ability to lift, transfer, and assist individuals as required by the position.
- May require weekends, holidays, and flexible scheduling based on individual needs.
- Frequent standing, walking, bending, lifting, and assisting individuals with mobility needs.
- Ability to safely perform transfers and other physical support activities.
- Ability to accompany individuals in various community settings and activities.
