Physical Therapist (PT) – Laurens, IABlue Stone Therapy is seeking a Physical Therapist to provide rehabilitation services to adult and geriatric patients at Laurens Care Center in Laurens, IA.This role focuses on evaluating and treating adult and geriatric patients in a skilled nursing and post-acute rehabilitation setting. The Physical Therapist is responsible for patient assessments, care plan development, and implementation of treatment programs designed to improve gait, balance, mobility, strength, range of motion, pain management, and overall functional independence.$5,000 sign-on bonus available for eligible full-time candidates.What You'll Do:Provide skilled physical therapy services to adult and geriatric patients in a skilled nursing and post-acute rehabilitation settingPerform patient evaluations and develop plans of careDeliver therapy focused on gait training, balance, mobility, strength, range of motion, endurance, and pain managementMonitor patient response to treatment and adjust interventions in collaboration with the therapy teamMaintain appropriate patient schedules per established guidelinesComplete timely and accurate documentation to support quality care and reimbursement requirementsCollaborate with interdisciplinary team members and facility staff to support positive patient outcomesWhat You Bring:Graduate of an accredited Physical Therapy programCurrent PT (or eligibility for licensure) in the applicable stateExperience in skilled nursing, long-term care, or post-acute rehabilitation preferredStrong clinical reasoning, communication, and organizational skillsCommitment to patient-centered care and professional standardsWhy Blue Stone Therapy?We believe exceptional care begins with exceptional clinicians.
