Primary Care Opportunity making over $300,000 in Birmingham, AL \n

COMPENSATION: $260,000 BASE TOTAL POTENTIAL $312,000 + Full Benefits \n

Role: \n

Compensation: Base $260,000 + up to 20% of base salary in bonus’ \n

Bonus’ based on: 1. Patient satisfaction rating 2. Living the company’s values 3. HEDIS compliance 4. Reducing hospital sick days \n

Primary Care – Outpatient \n

Start date: New center opens May 2025 \n

Value Based Care Model \n

Patient population: Seniors with 3-5 complex diseases (Hearth disease, diabetes, hypertension etc.) \n

Average patients per day: 10-12 \n

Panel size: Max of 650 \n

Shifts: Monday-Friday 8a-5p \n

Call Schedule: Based on number of providers in the market (if 32 physicians in market--call 1:32wks) \n

Physicians have their own care team with MA/scheduler/ referral specialist and full support from leadership \n

Requirements: \n

\n

MD or DO \n

Board Certified/Board Eligible FM/IM (required) \n

Fellowship in Geriatrics a plus but not required \n

Current, unrestricted, active license to practice medicine \n

Current DEA registration \n

Proficient PC skills in office procedures: suturing, joint injections, skin biopsies, basic wound care \n
