LPN Intake Coordinator

Managing the intake process for new patients, the full-time LPN Intake Coordinator will facilitate referral management, insurance verification, and initial assessment scheduling in a remote setting. Key responsibilities Manage the intake process, including receiving referrals, verifying insurance, and scheduling assessments Serve as the primary contact for patients and healthcare professionals regarding Home Health and Hospice services Ensure accurate documentation of patient information and collaborate with the interdisciplinary team to develop care plans Required qualifications Associate's Degree or Certification in Nursing Current Licensed Practical Nurse license in the state of service or multi-state license through the enhanced Nurse Licensure Compact (eNLC) Two years of Home Health and/or Hospice experience Two years of experience in utilization review/chart audit Bachelor's degree in nursing is preferred
