Adjust treatment plan to achieve maximum results in independence and/or function. Identify and assist the patient and/or family to utilize appropriate community resources to achieve measurable objectives. Utilize special skills and judgment to resolve adverse social and emotional responses connected with the injury/illness. Provide direct social work intervention and instruct and counsel the patients and family members in treating and coping with social and emotional responses connected with the injury/illness. Regularly re-assess effectiveness of treatment plan, progress of patient towards goals and the need for modifying goals or treatment. Communicate patient’s needs and progress to the treatment team, physician, patient, family members, and others as applicable. Consult with medical providers regarding patient evaluation & treatment, progress of the patient & discharge from treatment. Observe/document and report changes in the condition/status of the person receiving services to the Clinical Coordinator, Program Manager or DCM, attending physician and other treatment team members. Maintain and submit evaluation, treatment and billing documentation in the format and timelines required by the Company, facility, funding source, local, state and federal regulations. Adhere to standards of the Company's Code of Business Conduct, policies, HIPAA and Corporate Compliance programs and appropriate professional practice standards and applicable state/federal laws. Perform other duties as assigned. Qualifications Master's Degree in Social Work or Marriage and Family Therapy (LCSW, MSW, MFCC, or LPC) Current Social Work or Marriage and Family Therapist licensure/certification in the state(s) of practice required by individual states. A minimum of one year of experience in a rehabilitation setting serving individuals who have experienced a traumatic brain injury, spinal cord injury or other acute diagnosis. Current BLS (Basic Life Support) First Aid and CPR certifications. Demonstrates effective and professional interpersonal, verbal, and written communication skills. Able to work independently and part of a team. Demonstrates strong organization skills and attention to detail. Additional Job Information Ability to lift fifty (50) pounds. Moving, lifting, or transferring of patients may involve lifting of up to 100 pounds. Ability to stand for extended periods. Fine motor skills. Visual acuity. Duties require extensive standing and walking. Requires lifting, positioning, pushing, and/or transferring patients.
