LVN / LPN - LTC/SNF

Job Summary:

Responsibilities:

• 
  Perform comprehensive patient assessments and prioritize care needs.
  
  
• 
  Administer medications and treatments in accordance with physician orders.
  
  
• 
  Communicate relevant patient information to MD/NP and document accurately.
  
  
• 
  Assist residents with activities of daily living, including ADL support.
  
  
• 
  Provide specialized care such as gastrostomy tube, urinary catheter, wound care, and CPAP management.
  
  
• 
  Ensure compliance with facility policies and infection control protocols.
  
  

Qualifications:

• 
  Valid LPN license with experience in a long-term care or SNF setting.
  
  
• 
  Proven ability to perform clinical assessments and deliver direct patient care.
  
  
• 
  Knowledge of wound care, gastrostomy, urinary catheter, and respiratory treatments.
  
  
• 
  Strong communication skills for effective interaction with healthcare team and residents.
  
  
• 
  Ability to adapt to varied shifts based on census and facility needs.
  
  
• 
  Proof of flu vaccination or valid medical/religious exemption required before start.
