PT - Physical Therapist - PRN (Fannin & Grayson county and Anna area) McKinney, TX Description Physical therapists provide treatment for physical impairments, functional limitations or disabilities. The treatment is in accordance with their plan of care. Limited supervision is required. Essential Functions Conduct therapy practice within defined standards of care according to APTA guidelines and practice act. Design and implement a Plan of Care for the patient and significant others based upon a thorough assessment. Provide direct patient care, evaluate and re‑assess patient’s functional needs to ensure optimal outcomes, consult with other specialists as needed. Ensure continuation of therapeutic plan by designing home exercise programs; instruct patients, families, and caregivers in home exercise programs; recommend and provide assistive equipment as needed. Evaluate outcomes of care with appropriate discharge planning. Provide direction and leadership to PTA, aides, and students. Make visits as scheduled and participate in on‑call rotation as required. Develop coordination and cooperation among other health‑care disciplines to provide quality patient care. Maintain patient rights and dignity. Complete and submit documentation timely and accurately according to policy. Communicate effectively with patients, families, and the healthcare team. Enhance professional development through participation in educational programs, current literature, in‑service meetings and workshops. Additional Responsibilities Perform other duties as assigned or requested. Conform to all applicable Agency policies and procedures. Participate actively in continuing education and in‑services. Maintain confidentiality of patient information and business trade practices. Assume accountability for reporting incidents and complaints according to Agency policy. Knowledge / Skills / Abilities Ability to supervise in accordance with Agency’s policies and applicable laws. Ability to respond to common inquiries or complaints, regulatory agencies, or members of the business community. Organizational and leadership abilities. Cooperative attitude. Advanced written and verbal interpersonal communication. Advanced math skills related to patient care. Information Management Treat all information and data within the scope of the position with appropriate confidentiality and security. Cooperate fully in all risk management activities and investigations. Keep abreast of changes in health care law. Maintain Agency/program compliance with local, state and federal laws as well as state accreditation standards. Minimum Position Qualifications Education: Graduate of an accredited Physical Therapy program. Experience: Two (2) years of experience as a physical therapist in the last five (5) years; must have adult home health experience; HCHB experience is a plus. Training: Appropriate competency assessments completed. License / Certification: Current licensure; CPR certification; current driver’s license and proof of auto liability insurance. Environmental Conditions Works in patient’s home in various conditions, possible exposure to blood and body fluids and infectious diseases. Ability to work flexible schedule, ability to travel locally, some exposure to unpleasant weather. Moderate noise level; tasks may involve moderate stress and emotional demands. Physical Requirements Prolonged standing and walking required. Ability to move up to 50 lbs. and move patients. Requires working under some stressful conditions to meet deadlines, to identify patient needs, to make quick decisions and resource acquisition; meet patient/family psychosocial needs. Requires hand‑eye coordination and manual dexterity. Ability to utilize durable medical equipment in the home.
