Nurse Navigator, Total Joint (MUVE) Program

Job Summary Candidates will provide pre and post op education to patients related to total joint surgeries at Surgery Center of the Northland. The role will manage the surgical patient preparation for surgery and post op education. Individuals will assist in the screening and assessments of patients based on pre‑op history and physical and lab work. They will coordinate care with physicians, ensuring communication and preparation is streamlined. As needed, they will provide patient‑specific education on diagnostic testing, diagnosis, and treatment plan and will elevate concerns to anesthesiologists and/or surgeons. Collects information and data on total joint patients for reporting purposes. Maintains excellent communication and follow‑up with patients. Establishes and maintains relationships with providers. Oversees and aids with discharge planning, coordinating patient’s discharge needs with additional services as necessary. This position has guaranteed full‑time hours. Job Responsibilities Coordinates with anesthesia, primary care physician, and/or surgeon to ensure appropriate candidate for outpatient setting based on patient’s medical conditions prior to surgery. Collaborates with members of the medical team in the ASC and surgeons’ offices. Acts as a liaison between the patient, patient’s family, physicians, nursing, and ancillary contracted services. Reviews pertinent information about the patient’s medical, psychosocial, and functional status. Works within criteria set by the pre‑admission/registration process; sends appropriate referrals to anesthesia/surgeon for final determination when information warrants. Provides necessary information and education to the patient and support person for the following: pre‑operative, intra‑operative, discharge, and post‑discharge processes. Job Requirements – Required 3+ years RN experience in a hospital or ASC. 2+ years clinical orthopedic experience. Active RN license as required by state. BLS, ACLS, and PALS certification. Excellent interpersonal skills and written and oral communication required. Preferred BSN. Previous patient navigation or case management experience.
