Licensed Clinical Social Worker (LCSW)

Role: Provides direct clinical social work services, counseling, discharge planning, and case management to patients and families in a hospital setting. Collaborates with healthcare teams to develop and evaluate individualized psychosocial care plans, monitor progress, and address barriers to care. Facilitates integration with community resources, supports health management across the continuum of care, and ensures documentation complies with policies. Demonstrates strong assessment, critical thinking, organizational, and follow-through skills. Works effectively with diverse populations and maintains professional growth. Key Responsibilities Conduct psychosocial assessments and formulate care plans Provide counseling and emotional support to patients and families Develop discharge plans and coordinate community-based resources Collaborate with interdisciplinary healthcare team members Monitor patient progress and communicate changes Complete accurate documentation in medical records Support quality improvement and organizational goals Float to other departments as needed Qualifications & Skills Licensed Clinical Social Worker (LCSW) license required Strong psychosocial assessment, critical thinking, and clinical decision-making skills Experience working with patients coping with loss and grief Excellent organizational and follow-through skills Ability to work with diverse populations Additional Requirements: BLS, Pharmacology, Skills Checklist, and Specialty Exam for onboarding. #J-18808-Ljbffr
