Ok to license • DEA: AZ registered • CSR: not required • Certifications Required: ACLS • Minimum Experience Required: 1 year Credentialing Timeframe: 30 days OT: 1.5 normal rate OT Billing Rule: after scheduled shift Holiday: 1.5x normal rate - New Year’s Day Memorial Day Independence Day Labor Day Thanksgiving Day Christmas Day [00:00 – 23:59].
