Family Practice/Primary Care Physician Assistant

We are currently looking for a Family Medicine nurse practitioner AND/OR physician assistant to join our Family Medicine private practice in Lincoln, CA a suburb of Sacramento. Key Responsibilities: Comprehensive Primary Care: Provide preventive care including annual physicals, well-child visits, immunizations, and sick visits for both children (ages 6 and older) and adults. Acute Care Services: Offer urgent care, pediatric care, short-term stabilization, and post-hospital follow-up care designed to keep patients safe, independent, and out of the hospital. Chronic Disease Management: Manage acute and chronic conditions, including providing medication therapy management, diet and nutrition counseling, and mobility/home medical equipment assessments. Geriatric & Palliative Care: Provide specialized adult primary care and patient observation tailored to our senior citizen population. Collaborative Practice: Work collaboratively with our clinical team and network of specialists to ensure comprehensive continuity of care. Work Life Balance: Predictable work schedule, flexible full or part-time option, no after hours practice call. Qualifications: 2+ years of primary care experience required, no new grads considered Must love building relationships with patients, value bedside manner, and possess excellent clinical skills Must comply with HIPAA rules and regulations State licensure and board certification is required About Lincoln, CA : Lincoln, California, has a population of 56,000 residents and is located just outside the Sacramento metropolitan area. The salary range for this role is $70.00 per hour in base pay and exclusive of any bonuses or benefits.
