Nurse Practitioner for our Skilled Nursing Facility (SNF)

Compensation: $100,000 - $170,000 Annual Salary

Position Description

We are looking for a dedicated Nurse Practitioner to serve as the primary care provider for our skilled nursing facility in St. Albans, VT. This is a full-time, in-person position, Monday through Friday during daytime hours, with no nights, no weekends, and no on-call. You will manage a panel of long-term care and short-stay rehab residents, handling chronic disease management, acute changes in condition, and coordination with the facility's nursing and therapy teams.

Qualifications of the Nurse Practitioner:

• Active and unrestricted Vermont Nurse Practitioner license in good standing
• Active DEA registration
• Board certification as an FNP, AGNP, or AGPCNP
• Experience in skilled nursing, long-term care, or geriatrics preferred

Responsibilities of the Nurse Practitioner:

• Provide primary care to residents, including physical exams, assessments, and medication management
• Diagnose and treat chronic and acute conditions within the NP scope of practice
• Order and interpret labs and diagnostic tests, and adjust care plans accordingly
• Collaborate with facility nursing staff, therapy, and the interdisciplinary team
• Maintain accurate and compliant clinical documentation
