Job Summary:
Registered Nurse (RN), Home Healthcare, Newton, Massachusetts. This position involves assessing patient conditions, implementing care plans, and providing ongoing education and support to patients and their families within community health practice standards. Candidates are required to bring their licenses on the first day of employment.

Job Details:

• Certifications, Education Requirements, Degree Requirements, Location Requirements, and Years of Experience: 1 year Certified Home Care experience, 1 year Case Management Experience, current nursing license, located in Massachusetts, with no minimum years of experience required.
• Schedule: Days shift, 8 hours shift, Minimum of 40 hours per week.
• Additional Information: The role requires the ability to travel to cases as assigned. Weekend rotations are scheduled once every 6 weeks. All applicants must have at least 1 year of experience specifically within a certified home health agency; experience in hospice, private care, or as a standalone case manager does not meet the qualification criteria.
