Integrated Behavioral Health Clinician (BHC) - Licensed Clinical Social Worker (LCSW), Licensed Professional Counselor, Licensed Marriage & Family Therapist (LMFT) 

Praxis Health is seeking a skilled and compassionate Behavioral Health Clinician (LCSW, LPC, LMFT) to join our integrated care teams in beautiful Eugene, Oregon. This is an exciting opportunity to be part of a fast-paced, team-based environment that truly embodies whole-person care.

Unlike traditional outpatient therapy, our model follows the Primary Care Behavioral Health (PCBH) approach — consultative, collaborative, and focused on brief, functional interventions that help patients improve overall health outcomes.

Position Summary:
The Integrated Behavioral Health Consultant is a core member of the primary care team who works collaboratively with medical providers and support staff to address patients' behavioral health needs within the primary care setting. This role is embedded in the primary care clinic and provides brief, evidence-based interventions, psychoeducation, care coordination, and consultation to support patients experiencing a wide range of behavioral, emotional, and physical health concerns.

Ideal Candidate:
The ideal candidate has experience in a fast-paced medical environment, preferably within Primary Care, and understands the principles of integrated care, and is comfortable delivering brief, goal-oriented interventions. Strong communication, flexibility, and a proactive, collaborative approach are essential.

What We Offer:

• Salary : $90,000 - $115,000 per year DOE & Licensure

Schedule

• 36 per week patient contact hours/ week + ~4 admin hours/week

Minimum Qualifications

• Educational Qualifications : Master's degree in Social Work, Psychology, Counseling, or a closely related field.
• Licensure Requirements : Current licensure as a Licensed Clinical Social Worker (LCSW, LPC, LMFT))
• Experience working in an integrated care setting preferred but not required.
• Strong clinical assessment and intervention skills with a focus on brief, solution-focused therapy.
• Ability to work collaboratively within a multidisciplinary team and communicate effectively with primary care providers.
• Comfortable with using telehealth technology for remote patient care.
• Flexible and adaptable to changing work environments and patient populations.
• Commitment to providing culturally competent and trauma-informed care.
• Will attend work regularly and on time, as scheduled.

General Physical Requirements

• While performing the duties of this Job, the employee is regularly required to stand; walk; use hands to finger, handle, or feel; reach with hands and arms and talk or hear.
